You may also want to fill out one of our questionnaires related to your particular complaint prior to you visit. For example if you are an adult being seen for a sleep disorder you will need to fill out the “Sleep Questionnaire”.   You can access other forms here.

NEW PATIENTS

When you are a new patient to our office or if it has been several years, there are a few forms that need to be filled out before you see the doctor.

Please bring your, insurance card, list of medications and any pertinent records to your visit. Remember, some insurance plans require a prior authorization from your primary doctor. It is your responsibility to make sure the authorization or referral has been approved. If not, we will try to contact the insurance for an urgent authorization, but the appointment may need to be cancelled or you may be responsible for the payment for the visit.
